What is needed to make integrated care work?

The EU-funded research project INTEGRATE did four integrated care case studies in Europe, in the areas of chronic obstructive pulmonary disease, mental health, geriatric conditions and diabetes. The studies showed that integrated care improves patient experiences, generates better care outcomes and provides cost-effectiveness. But what constitutes good quality integrated care provision, and how can integrated care systems be most effectively built? This video explains.
